Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients during the night shift
• Administer medications and treatments accurately and document care in the medical record
• Monitor patient status, report changes to the care team, and escalate concerns promptly
• Collaborate with nurses, CNAs, and interdisciplinary staff to ensure continuity of care
• Perform assessments, wound care, and routine procedures per facility policies
• Support residents' comfort and safety through thoughtful bedside care and communication

Qualifications

• Active LPN license (Licensed Practical Nurse) — required
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Recent clinical experience in a long-term care, rehab, or acute care setting preferred
• Ability to work overnight shifts and adapt to per diem scheduling
• Up-to-date immunizations and clear background/health screening per facility policy
• Strong communication skills and a patient-centered approach

Education

• Graduate of an accredited Practical Nursing / LPN program (diploma or certificate)

Knowledge & Skills

• Knowledge of medication administration and basic nursing assessments
• Familiarity with long-term care and rehabilitation nursing practices
• Comfortable with electronic charting and documentation best practices
• Ability to prioritize tasks, triage concerns, and work independently overnight
• Strong interpersonal skills for working with residents, families, and care teams
