Roles & Responsibilities

• Provide skilled physical therapy care to home health patients, focusing on mobility, safety, and functional outcomes
• Conduct thorough initial and ongoing patient assessments and develop individualized treatment plans
• Deliver patient- and family-centered education to support home exercise programs and safe independence
• Document visits accurately and timely in the electronic health record (Epic preferred)
• Collaborate with nurses, OTs, SLPs, and case managers to coordinate comprehensive care
• Perform required competency testing and skills checklists as part of Trinity-specific processes
• Adhere to infection control and safety protocols during all home visits

Qualifications

• Active Physical Therapist license with successful license/certification verification
• BLS (Basic Life Support) certification required
• Home health physical therapy experience preferred
• Ability to complete Trinity specialty competency testing and skills checklists
• Willingness to work weekends as needed
• Ability to pass required health screenings and immunizations (fit test, drug screen, MMR, Hep B, TB screening or IGRA, Varicella, Tdap, COVID-19, influenza)

Knowledge & Skills

• Strong patient assessment and intervention skills for home-based care
• Skilled in mobility training, fall prevention, and ADL-focused rehabilitation
• Comfortable with clinical documentation and charting; Epic experience preferred
• Excellent communication and patient education abilities
• Familiarity with infection control, safety checks, and home environment assessments
• Ability to work independently and manage caseloads in the community
