Roles & Responsibilities
- Verify medications and perform accurate medication reconciliation to keep patients safe
- Compound and verify sterile IVs using Dose Edge procedures and best practices
- Provide therapeutic drug monitoring and dose adjustments for vancomycin, aminoglycosides, and anticoagulants
- Supervise and mentor pharmacy technicians, including Pyxis and Pak Edge checks
- Perform medication verification, first-dose checks and manage emergency med boxes
- Collaborate with clinical teams on pain consults and medication-related recommendations
- Document interventions in the EMR (Meditech) and communicate effectively with providers
Qualifications
- Degree from an accredited Pharmacy School (required)
- Licensed to practice Pharmacy in Missouri (MO) — active, in good standing
- Minimum 2 years of relevant inpatient pharmacy experience
- ACLS and BLS preferred
- NICU experience is a plus
- Comfortable using Dose Edge, Pyxis, Pak Edge and Meditech
Education
- Degree from an accredited Pharmacy School
Knowledge & Skills
- Strong knowledge of inpatient pharmacy workflows and sterile compounding
- Clinical dosing expertise for vancomycin, aminoglycosides and anticoagulants
- Experience with medication verification workflows and Pyxis management
- Familiarity with Dose Edge verification processes and Pak Edge systems
- Proficient with Meditech EMR documentation
- Excellent communication, teamwork and supervisory skills
