Roles & Responsibilities

• Provide thorough physical therapy evaluations and develop individualized home health care plans
• Deliver skilled PT interventions in patients’ homes, focusing on functional mobility and safety
• Document visits accurately and promptly in HCHB to ensure continuity of care
• Coordinate care with nurses, social workers, physicians and other therapists to optimize patient outcomes
• Educate patients and caregivers on home exercise programs, fall prevention and safe transfers
• Manage a daily caseload efficiently, prioritizing patient-centered care and timely visits
• Serve as an advocate for patients in the community, addressing barriers to care and resources

Qualifications

• Valid Physical Therapist license (required) — ability to practice in North Carolina
• Minimum 2 years of clinical experience (as listed in the job posting)
• Home health experience required for therapy positions
• Experience with HCHB documentation preferred or prior EMR experience
• Valid driver’s license and reliable vehicle for patient home visits
• Strong communication skills and ability to collaborate with interdisciplinary teams
• Comfort working independently in community/home settings

Knowledge & Skills

• Skilled in performing home-based functional assessments and developing goal-directed plans
• Proficient with documentation systems (HCHB) and clinical charting best practices
• Time management and route planning for efficient patient visits
• Patient and caregiver education techniques for home safety and exercise adherence
• Clinical reasoning for discharge planning and care coordination
• Infection control and safe handling practices in home environments
