Roles & Responsibilities

• Coordinate patient care across interdisciplinary teams to ensure smooth transitions
• Manage case assignments and drive timely discharge planning
• Perform comprehensive care coordination for acute patients
• Document accurately and efficiently in Cerner to maintain high-quality records
• Communicate clearly with patients and families to support care plans
• Collaborate with social work, physicians, and ancillary services to remove barriers to discharge
• Serve as a first-to-float resource, stepping into units as staffing needs arise

Qualifications

• Active California RN license required
• BLS Certification required
• LA City Fire & Safety certification required
• Minimum 2 years of relevant experience (acute care/case management)
• Prior California Dignity/CommonSpirit experience is mandatory
• Experience using Cerner for charting and care coordination
• Comfort floating to other units when needed

Education

• Active RN license in California (BSN not required)
• Associate degree, diploma in nursing, or equivalent nursing education acceptable

Knowledge & Skills

• Strong knowledge of care coordination and case management processes
• Familiarity with acute care workflows and discharge planning
• Proficient charting skills in Cerner
• Excellent communication and interdisciplinary collaboration skills
• Time management and prioritization in a fast-paced hospital environment
• Problem solving to help remove barriers to timely discharges
