Roles & Responsibilities

• Provide direct, compassionate bedside care to residents and patients
• Administer medications and treatments accurately and safely
• Perform timely patient assessments and document changes in condition
• Collaborate with the interdisciplinary team to coordinate care plans
• Support admissions, discharges and transfers with professionalism
• Communicate clearly with patients, families, and staff to promote positive outcomes
• Maintain regulatory and facility documentation with attention to detail

Qualifications

• Licensed Practical Nurse (LPN) or T-LPN (as listed) — active and in good standing
• Ability to work the posted day shift hours (6:00 AM - 6:30 PM)
• Previous experience in long-term care, skilled nursing, or similar preferred
• Current BLS/CPR certification preferred or obtained upon hire
• Reliable, professional attitude with strong teamwork and communication skills

Education

• Graduate of an accredited practical nursing program / LPN diploma
• State licensure as an LPN (required)

Knowledge & Skills

• Solid medication administration and documentation skills
• Patient assessment and vital signs monitoring
• Wound care basics and infection control awareness
• Familiarity with care planning and charting best practices
• Strong interpersonal skills and patient-family centered care approach
