Roles & Responsibilities

• Provide direct patient care to residents in the SNF with compassion and professionalism
• Perform resident assessments and make rounds, especially for new or ill residents
• Document assessments and care accurately and promptly per facility policy
• Receive and give clear, thorough report/hand-off to nursing staff
• Notify nurses of any changes in resident condition or new care needs
• Answer call lights promptly and assist residents with activities of daily living
• Foster positive relationships with residents, families, physicians and co-workers
• Float to other units as needed and take on other duties as assigned to support the team

Qualifications

• Valid California CNA license required
• Experience providing direct patient care in a long-term care or SNF setting preferred
• Strong communication skills and ability to work collaboratively as part of a care team
• Willingness to work weekends and holidays and float to other units as needed
• Ability to document assessments and care per facility policies

Knowledge & Skills

• Resident assessment and recognizing changes in condition
• Accurate documentation and charting per facility policy
• Effective reporting and hand-off communication with nursing staff
• Direct patient care skills and answering call lights promptly
• Teamwork, professional communication, and working with multidisciplinary teams
• Ability to follow care plans and directives from licensed nursing staff
