Roles & Responsibilities

• Perform comprehensive clinical assessments and create individualized care plans
• Provide expert pain and symptom management consistent with each patient’s hospice plan of care
• Educate and support patients and families about the disease process, self-care, and end-of-life planning
• Coordinate care and collaborate closely with physicians, social workers, chaplains and volunteers
• Deliver skilled nursing care during home visits and respond promptly when on-call
• Maintain accurate, timely documentation in compliance with state, federal and organizational policies

Qualifications

• Current Registered Nurse (RN) license in Utah or compact state RN license
• Minimum 3 years of nursing experience (hospice, home health or palliative care preferred)
• Current BLS certification
• Valid driver’s license and reliable transportation for home visits
• Ability to respond within 45 minutes while on-call
• Willingness to participate in weekend and holiday coverage as scheduled

Education

• Active Registered Nurse (RN) license (UT or compact)
• Current BLS certification

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Proficient in pain and symptom management for terminally ill patients
• Excellent documentation and compliance practices
• Skilled at patient and family education with a compassionate approach
• Experience with HospiceSoft and/or Careficient EMRs is a plus
• Effective interdisciplinary collaboration and communication skills
• Organizational skills for caseload management and timely charting
