Roles & Responsibilities

• Provide compassionate, high-quality bedside care to assigned residents as an LPN
• Administer medications and treatments accurately and document per facility policy
• Perform routine assessments and promptly communicate changes in condition to the care team
• Collaborate with RNs, providers, and support staff to implement individualized care plans
• Support admissions, discharges, and transfers while maintaining thorough documentation
• Contribute to a safe, clean, and respectful environment for residents and families

Qualifications

• Valid Licensed Practical Nurse (LPN) license for practice in Pennsylvania
• Current Basic Life Support (BLS) certification (or willing to obtain prior to start)
• Comfortable working night shift hours and every other weekend coverage
• Ability to pass required background checks and health screenings for travel assignments
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited LPN/LVN program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Skilled in patient assessments and recognizing changes in condition
• Experience with wound care, ADL assistance, and IV/PO medication routes as applicable
• Proficient at electronic charting and maintaining accurate clinical records
• Excellent interpersonal skills for working with residents, families, and multidisciplinary teams
