Roles & Responsibilities

• Assist licensed physical therapists in delivering compassionate, evidence-based care across outpatient, inpatient, swing bed, and home health settings
• Implement treatment plans and therapeutic exercises to help patients regain mobility, strength, and function
• Monitor patient progress, document outcomes accurately in the electronic medical record, and communicate changes to the care team
• Collaborate with interdisciplinary team members to coordinate care and ensure seamless patient transitions
• Educate patients and families on home exercise programs, safety strategies, and self-management techniques
• Maintain a safe, professional environment and adhere to clinic policies and regulatory standards

Qualifications

• Active LPTA license in good standing (required)
• CPR certification (required)
• Comfortable managing a mixed caseload: roughly 60-70% outpatient and 30-40% inpatient/swing bed/home health
• Strong interpersonal skills and the ability to function effectively as part of a team
• Basic computer skills and experience with electronic documentation
• Bilingual Spanish a plus — helpful for connecting with a broader patient population

Knowledge & Skills

• Clinical knowledge of therapeutic exercise, modalities, gait training, and functional mobility
• Skilled in patient assessment, documentation, and treatment progression
• Familiarity with outpatient and inpatient workflows, and home health best practices
• Effective communication and patient education techniques
• Proficient with EMR documentation and basic computer applications
• Team-oriented mindset and strong interpersonal skills
