Roles & Responsibilities

• Interview patients and families to gather thorough psycho-social information and assess impact of illness
• Develop and contribute to interdisciplinary plans of care and coordinate discharge planning
• Prepare clear, compliant treatment plans and deliver crisis counseling when needed
• Provide therapeutic support and education to patients and caregivers to assist with understanding medical recommendations
• Manage an assigned caseload and maintain timely, high-quality documentation in the electronic health record
• Collaborate with state and community agencies to secure resources for housing, financial assistance, and placement needs
• Handle referrals and consults from primary care and outlying clinics within scope of practice
• Advocate for patients, provide outreach, and link clients to essential community services

Qualifications

• Active Alaska LCSW required at time of submittal
• Licensed Master Social Worker (LMSW) noted in education section, but Alaska LCSW is required per posting
• NABCS Clearance and ICPA Clearance required
• Minimum of 2-4 years of social work experience in an inpatient or outpatient healthcare environment
• Flu vaccine required prior to assignment
• Must be at least 21 years of age
• Competence with electronic health records and technology; strong documentation skills
• Ability to complete fingerprint appointment and submit fingerprint cards promptly to avoid start delays

Education

• Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) (Alaska licensure required as specified)

Knowledge & Skills

• Psycho-social assessment and case management principles in integrated settings
• Crisis intervention and crisis counseling with calm, professional judgment
• Strong oral and written communication, empathic listening, and conflict resolution
• Familiarity with basic nutrition and tobacco cessation guidance
• Experience working with homelessness, addictions, and severe and persistent mental illness
• Comfort using EHRs, Google Drive, fax and phone systems
• Ability to coordinate care with multi-disciplinary teams and external agencies
• Customer service and recovery-oriented approaches when supporting patients and families
