Roles & Responsibilities

• Conduct thorough occupational therapy evaluations to assess functional needs and set meaningful goals
• Design and implement individualized treatment plans that optimize independence and quality of life
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Document patient progress and outcomes clearly and timely to support continuity of care
• Educate patients and families on strategies, adaptive equipment, and home programs to sustain gains
• Participate in onboarding and training activities to ensure high-quality, consistent service delivery

Qualifications

• Current Minnesota Occupational Therapy license or ability to obtain licensure in Minnesota
• Graduation from an accredited Occupational Therapy program and clinical fieldwork completion
• Strong evaluation and treatment planning skills in rehabilitation and outpatient settings
• Excellent communication and teamwork skills to collaborate with patients, families, and staff
• Ability to complete the required four-week onboarding period and adapt to clinic workflows
• Willingness to pass background checks and meet facility onboarding requirements

Education

• Graduation from an accredited Occupational Therapy program (entry-level OTD, MSOT, or equivalent)

Knowledge & Skills

• Skilled in functional assessments, ADL training, and adaptive equipment recommendations
• Knowledge of rehabilitation principles and community-based care approaches
• Proficient documentation habits and outcomes tracking
• Ability to provide compassionate, hands-on care with a patient-centered approach
• Comfort working with a variety of patient populations across the lifespan
