Contract Type: Travel Contract

Shift Type: Rotating 8- or 12-hour shifts (cycle scheduling). Possible shift times include 6:00 AM - 6:30 PM, 6:00 PM - 6:30 AM, 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, 10:00 PM - 6:30 AM. Schedule includes every other weekend and every other holiday; rotating day/night coverage.

Roles & Responsibilities

• Provide compassionate, resident-centered care and assist with activities of daily living
• Assist with bathing, dressing, mobility and toileting while preserving dignity
• Accurately document resident care and observations in PointClickCare
• Communicate changes in resident condition to the charge nurse and care team
• Support care plans for residents with CHF, COPD, dementia, Parkinson’s and diabetes
• Follow BLS and facility infection control procedures to keep residents safe
• Participate in unit orientation and collaborate with a close-knit team
• Contribute to a home-like atmosphere through respectful, ethical caregiving

Qualifications

• Minnesota CNA license required at time of submission
• Current BLS (Basic Life Support) certification required
• Minimum 2+ years of experience working in Minnesota nursing homes (LTC/SNF)
• PointClickCare (PCC) electronic charting experience required
• Comfortable with minimal IV medication exposure and working under charge nurse supervision
• Willingness to work rotating day/night shifts, every other weekend and holiday as scheduled
• Able to complete 2–4 shift unit-specific orientation

Knowledge & Skills

• Proficient with PointClickCare (PCC) charting and EMR workflow
• Experience in long-term care / skilled nursing facility operations
• Knowledge of common LTC diagnoses: CHF, COPD, Alzheimer’s, Parkinson’s, diabetes
• Strong resident safety, infection control and documentation practices
• Team-oriented communication with nursing leadership and interdisciplinary staff
• Ability to work both 8- and 12-hour shift rhythms and adapt to rotating schedules
