Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living
• Monitor and report changes in patient condition to nursing staff promptly
• Assist with safe transfers, ambulation, and mobility support
• Maintain accurate patient records and document care provided
• Support a clean, safe environment by following infection control and safety protocols
• Collaborate with the care team to ensure each resident’s dignity and comfort

Qualifications

• Active CNA certification in good standing (required)
• Current BLS/CPR certification preferred or willing to obtain
• Valid state-required immunizations and background checks as per facility policy
• Comfortable working with adults in a long-term care/rehab setting
• Strong communication skills and a caring, patient-focused attitude

Education

• High school diploma or equivalent preferred
• Completion of an approved CNA training program and active certification

Knowledge & Skills

• Proven ability to assist with ADLs: bathing, dressing, toileting, feeding
• Familiarity with basic vital sign measurement and reporting
• Knowledge of infection prevention and safe patient handling
• Good teamwork, time management, and observational skills
• Ability to follow care plans and direction from nursing staff
