Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide comprehensive nursing care to clients of all ages in home and hospice settings
Conduct admissions, evaluations, and develop individualized care plans
Collaborate with a team of homecare professionals and paraprofessionals to ensure coordinated care
Strive for the highest level of quality and compassion in every patient interaction
Document patient progress and communicate effectively with the healthcare team

Qualifications

Current active Vermont RN license
At least one year of prior clinical nursing experience
Compassionate and team-oriented approach to patient care
Ability to work independently and manage a diverse caseload

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Proficiency in electronic medical records (EMR)
Knowledge of home health and hospice regulations preferred
