Roles & Responsibilities

• Conduct screenings, clinical assessments, diagnoses, treatment planning, brief therapy, and continuing care for patients from birth through elder
• Lead and coordinate discharge planning for ALF and transitional placements to ensure safe, smooth transitions of care
• Facilitate or co-facilitate patient care conferences and collaborate with multidisciplinary teams and community partners
• Provide care coordination, client advocacy, skill-building, and support around complex issues such as Power of Attorney when relevant
• Deliver clinical support for chemical dependency and mental health needs, and make appropriate referrals
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and hospital billing standards
• Complete required provider enrollment and billing duties as assigned to support service delivery and reimbursement
• Participate in SEARHC Quality Management trainings and integrate best practices into client care

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
• Licensed as or in process of becoming an LCSW, LPC, or LMFT (Licensed Clinical Social Worker, Licensed Professional Counselor, or Licensed Marriage & Family Therapist)
• Valid State of Alaska driver’s license or ability to obtain within 6 months of hire
• Must be 21 years of age
• Clinical Competency assessment (SEARHC) required within 3 months of hire and recurrent every three years
• Ability to perform case management, crisis intervention, and effective clinical documentation
• Comfort working across the lifespan and collaborating with tribal health and community partners

Education

• Master's Degree in Social Work, Psychology, Counseling, or related discipline

Knowledge & Skills

• Discharge planning and care coordination across the continuum of care
• Clinical assessment, screening, diagnosis, and treatment planning
• Brief therapy and continuing care skills
• Thorough documentation practices that meet accreditation and hospital billing requirements
• Interviewing techniques and crisis intervention strategies
• Case management, patient advocacy, and skill-building approaches
• Familiarity with state, federal, and accreditation regulations
• Experience with provider enrollment processes and basic billing duties
• Ability to work effectively in a multidisciplinary team and support cultural wellness in tribal health settings
