Roles & Responsibilities

• Provide high-quality physical therapy services in patients' homes, delivering hands-on care and therapeutic interventions
• Perform comprehensive evaluations and develop individualized treatment plans that improve function and safety
• Document assessments, treatment plans, and progress notes accurately and timely (Epic preferred)
• Educate patients and caregivers on exercises, fall prevention, adaptive strategies, and home safety
• Collaborate with nurses, case managers, and other providers to coordinate care and optimize outcomes
• Complete required Trinity specialty competency testing and skills checklists to maintain clinical excellence
• Maintain compliance with infection control practices and agency policies while working in home settings

Qualifications

• Active Physical Therapy license (state licensure verification required)
• BLS certification required
• Home health experience preferred; comfortable providing care in patient homes
• Willingness to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to pass required health screenings and background checks (fit test, drug screen, immunizations, TB test or IGRA, etc.)
• Strong communication skills and ability to educate patients and family caregivers

Knowledge & Skills

• Skilled in patient assessment, treatment planning, and therapeutic exercise
• Knowledge of home safety evaluations and fall-risk mitigation
• Documentation proficiency; Epic experience preferred but not required
• Familiarity with infection control, mobility devices, and adaptive equipment
• Good clinical judgment, time management, and ability to work independently in the field
