Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments in patients’ homes
• Develop individualized, goal-driven treatment plans and update them as patients progress
• Deliver 5–6 patient visits per day with attention to safety, function, and patient education
• Document thoroughly and accurately in HCHB after each visit
• Coordinate care and communicate effectively with nurses, hospice teams, and other providers
• Advise patients and caregivers on home safety, adaptive equipment, and community resources
• Maintain reliable transportation for off-site patient visits and timely scheduling

Qualifications

• Current state Physical Therapist license (able to practice in North Carolina or obtain state approval)
• Minimum 2 years of clinical experience; home health experience required for this role
• Experience with HCHB documentation strongly preferred
• Hospice experience is a plus for candidates who will support end-of-life care
• Comfort managing a daily caseload of home visits and strong time-management skills
• Valid driver’s license and reliable vehicle for patient visits

Knowledge & Skills

• Proven ability to perform home health evaluations and create home-focused plans of care
• Skilled in patient education, fall prevention, mobility training, and ADL progression
• Familiarity with home health documentation standards and HCHB software
• Strong communication and interdisciplinary collaboration skills
• Ability to adapt treatments to variable home environments and equipment availability
