Roles & Responsibilities

• Deliver skilled occupational therapy interventions in a skilled nursing facility, helping residents regain independence
• Perform evaluations and implement individualized treatment plans that make a real difference
• Document progress and outcomes clearly and timely to support care continuity and compliance
• Collaborate with nurses, therapists, and the interdisciplinary team to optimize patient care
• Educate patients and families on strategies to improve function and safety at home
• Be flexible to float within the 25-mile radius as needed, with drive-time considerations honored

Qualifications

• Active COTA (Certified Occupational Therapy Assistant) licensure required; client will not wait for pending licensure
• Experience working in a Skilled Nursing Facility (SNF) setting preferred
• Not open to new graduates for this role
• Reliable transportation and willingness to float within a 25-mile radius (drive time accounted for)
• Current immunizations, background check, and facility-required clearances as applicable

Knowledge & Skills

• Strong foundational knowledge of occupational therapy interventions for geriatric and post-acute populations
• Skilled documentation and familiarity with SNF clinical processes
• Excellent communication and interdisciplinary teamwork skills
• Ability to prioritize caseloads and manage time in a fast-paced setting
• Comfort adapting treatment plans to meet patient and facility needs
