Roles & Responsibilities

• Assess patients' speech, language, cognitive-communication, and swallowing abilities using evidence-based evaluations
• Develop individualized treatment plans that set clear, achievable goals and track progress
• Deliver skilled therapy interventions and adapt approaches to meet each patient’s needs
• Maintain accurate, timely patient health documentation and progress notes
• Collaborate with nurses, physicians, therapists, and families to provide coordinated, person-centered care
• Ensure compliance with infection control practices and vaccination requirements to keep patients and staff safe
• Participate in competency testing and agency checklists to maintain high standards of care

Qualifications

• Active Michigan Speech License required
• ASHA certification (CCC-SLP) required
• BLS certification required; ACLS also listed as a required certification
• Valid driver’s license with photo ID and successful background check
• Completion of agency skills checklist, resume/application, and required documentation
• Up-to-date immunizations and test records (TB test, Hepatitis B record, Influenza vaccine or declination, COVID-19, MMR, Varicella, Tdap)
• Negative drug screen (minimum 10-panel) and required attestation and physical/health statement
• Certificate of Liability Insurance and records of nursing-related competencies/exams as requested
• Successful completion of Prime Core Competencies Exam, Prime Skills Checklist, and Prime Specialty Exam

Knowledge & Skills

• Clinical patient assessment and development of individualized therapy plans
• Strong documentation skills and familiarity with patient health records
• Infection control practices and vaccination compliance knowledge
• Therapeutic techniques for speech, language, cognition, and swallowing disorders
• Excellent communication and family-centered care strategies
• Ability to work collaboratively within an interdisciplinary team
