Roles & Responsibilities

• Lead and coordinate the Long-Term Care QAPI program to improve resident care and safety
• Plan, organize, and direct quality assurance activities and integration across departments
• Facilitate accreditation readiness and coordinate regulatory survey responses (CMS compliance)
• Conduct chart audits, data collection and analysis to identify trends and improvement opportunities
• Perform root cause analyses (RCA) and collaborate with Risk Management on incident investigations
• Design, implement, and monitor action plans to correct identified deficiencies
• Organize and facilitate quality committee meetings and maintain meeting documentation
• Partner with leadership, providers, and staff to coach, educate, and embed quality practices

Qualifications

• Current, full, active, unrestricted Registered Nurse (RN) license in Alaska (required)
• CPHQ or QAPI Certified Professional (QCP) or other LTC Quality Certification obtained within the last 3 years (required)
• Basic Life Support (BLS) - American Heart Association (required)
• Minimum of 2 years' experience monitoring quality and regulatory requirements and driving process change
• Demonstrated leadership in facilitating cross-functional teams and quality improvement initiatives
• Comfortable performing chart audits, using Datix or similar incident reporting systems, and working with data

Knowledge & Skills

• Strong knowledge of CMS regulatory requirements and accreditation standards across care settings
• Understanding of quality improvement concepts, data analytics, and performance measurement
• Skilled in medical chart review, clinical outcome analysis, and developing action plans
• Excellent verbal and written communication and the ability to present findings effectively
• Proficient with Microsoft Office (Word, Excel, PowerPoint) and experience presenting data
• Familiarity with infection control facilitation and safety practices in long-term care
