Roles & Responsibilities

Assess and treat patients with movement dysfunction or pain to restore function and reduce risk of adverse events
Develop and implement individualized treatment plans focused on mobility improvement, pain management, and fall risk reduction
Document all patient care thoroughly — evaluations, progress notes, daily notes, discharges, and insurance communications
Coordinate discharge planning to support safe transitions and improved functional outcomes
Communicate effectively via phone and in-person with patients, families, insurance companies, rehab staff, and supervisors
Handle insurance approval/denial interactions and related documentation to support patient care continuity
Support clinic operations by maintaining clean treatment spaces, assisting with laundry and basic facility tasks, and performing other duties as requested

Qualifications

Licensed Physical Therapist in Wisconsin (or eligible for licensure)
Degree in Physical Therapy (Doctor of Physical Therapy preferred or equivalent)
Proven experience in patient assessment, treatment planning, and discharge planning
Strong documentation skills including evaluations, progress notes, and daily notes
Comfortable managing insurance communications and approvals/denials
Excellent verbal and phone communication skills and ability to collaborate with multidisciplinary teams

Education

Degree in Physical Therapy (DPT preferred)

Knowledge & Skills

Pain management techniques and therapeutic exercise prescription
Fall risk assessment and prevention strategies
Mobility improvement and functional training
Comprehensive patient evaluation and reassessment
Accurate clinical documentation and progress note writing
Familiarity with insurance approval/denial processes and effective phone communication
