Roles & Responsibilities

• Evaluate patients across inpatient, swing bed, outpatient, and skilled nursing settings and develop individualized treatment plans
• Deliver evidence-based physical therapy treatments in accordance with physician orders and state regulations
• Document clinical findings, progress, and discharge status accurately and timely in the EMR
• Collaborate with physicians, nurses, and interdisciplinary teams to coordinate care and optimize patient outcomes
• Perform patient/family education to support safe discharge and ongoing recovery
• Use strong clinical reasoning to manage diverse diagnoses and adapt treatment plans as needed

Qualifications

• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA Basic Life Support (BLS) certification
• 1+ year experience in acute care, swing bed/skilled nursing, or outpatient settings
• Strong documentation, communication, and clinical reasoning skills
• Proficiency with computers and EMR systems

Education

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Skilled in patient evaluation, treatment planning, and execution across care settings
• Experienced with inpatient care, swing bed/skilled nursing, and outpatient therapy
• Comfortable with clinical documentation and EMR workflows
• Solid problem-solving abilities and attention to detail
• Effective interdisciplinary communication and collaboration
