Roles & Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and design personalized treatment plans
• Deliver evidence-based physical therapy interventions to improve strength, mobility, and independence
• Educate patients and families on therapeutic exercises, safe movement, and home modifications
• Collaborate closely with occupational therapists, nurses, physicians and the interdisciplinary team to coordinate care
• Monitor patient progress, document outcomes, and adjust treatment plans to ensure optimal results
• Maintain accurate, timely documentation in accordance with facility policies and regulatory standards
• Ensure care plans align with best practices and regulatory requirements to maximize patient safety and quality

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in California (state licensure required)
• Experience working in a skilled nursing facility (SNF) or similar setting preferred
• Strong communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of a collaborative care team
• Compassionate, patient-focused approach with attention to detail in documentation

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program

Knowledge & Skills

• Patient assessment and evaluation
• Treatment planning and therapeutic exercise design
• Therapeutic modalities and use of adaptive equipment
• Mobility improvement and fall prevention strategies
• Accurate clinical documentation and progress tracking
