Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to improve function and quality of life
• Develop and implement individualized treatment plans based on assessment and patient goals
• Collaborate with an interdisciplinary team to coordinate seamless care
• Document clinical care accurately and timely in accordance with facility and regulatory standards
• Participate in care conferences and communicate progress to residents and families
• Contribute to quality improvement initiatives that enhance patient outcomes

Qualifications

• Active California Physical Therapist license required
• Strong clinical skills in geriatric and post-acute rehabilitation
• Skilled nursing or post-acute experience preferred
• Excellent communication and interpersonal skills
• Dependable, flexible, and team-oriented approach to care
• Commitment to delivering high-quality, compassionate therapy
