Roles & Responsibilities

• Establish collaborative practice relationships with oncologists to optimize inpatient and outpatient chemotherapy and supportive care plans
• Round with Oncology Fellows, NPs, and physicians to recommend medication adjustments and monitoring based on evidence-based guidelines
• Review and maintain oncology care plans in the Electronic Health Record to ensure guideline-consistent therapy
• Provide patient-specific medication education using teach-back for oral oncolytics and IV chemotherapy
• Serve as a subject matter expert by answering drug information queries and supporting oncology staff education
• Precept pharmacy residents and students and contribute to their clinical training
• Lead initiatives to improve safety, efficacy, and quality of cancer care within the Oncology Service Line
• Promote cost-effective drug therapy and identify opportunities for cost avoidance and formulary optimization

Qualifications

• Doctor of Pharmacy (PharmD) required
• Licensed Pharmacist (active pharmacy license required)
• Board Certified Oncology Pharmacist (BPS) expected/required per posting
• Minimum of three years hospital experience with clinical pharmacy training and three years of oncology practice experience
• PGY-1 residency or equivalent clinical experience required; PGY-2 Oncology residency or equivalent oncology experience preferred
• Demonstrated experience precepting residents/students and providing multidisciplinary education

Education

• Doctorate in Pharmacy (PharmD) required
• PGY-1 residency or equivalent clinical pharmacy practice experience required; PGY-2 Oncology residency preferred

Knowledge & Skills

• Strong oncology pharmacotherapy knowledge and clinical rounding experience
• Proficient with Electronic Health Record (EHR) review and oncology care plan management
• Skilled in medication monitoring, supportive care optimization, and patient education
• Experience with evidence-based guidelines and formulary/cost-effectiveness considerations
• Comfortable answering drug information questions and serving as a clinical resource
