Location: Utica, NY
Shift Type: Day shift, 6:30 AM - 2:30 PM, every other weekend required, On-call: None
Candidate Pay Rate: $56.1 - $39.27 per hour

Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in a 40-bed long term care unit
- Administer medications and treatments safely and accurately
- Assess and monitor residents' conditions, reporting changes to the interdisciplinary team
- Document care and clinical observations clearly and thoroughly in the EMR
- Collaborate with nursing, therapy and support staff to develop and follow individualized care plans
- Support residents with dementia and memory care needs using best-practice approaches
- Assist with admissions, discharges and transfers to ensure smooth transitions
- Promote a safe environment by following infection control and safety protocols
Qualifications
- Active New York LPN license required
- Current BLS certification required
- Minimum 1 year of LPN clinical experience
- Long term care experience preferred
- Experience with PointClickCare EMR is a plus
- Strong clinical judgment and excellent documentation skills
Education
- Graduate of an accredited LPN/LVN program (or equivalent practical nursing education)
Knowledge & Skills
- Medication administration and medication safety practices
- Skilled clinical assessment and resident monitoring
- Charting and electronic documentation (PointClickCare familiarity preferred)
- Geriatric care, dementia care, and memory care approaches
- Good communication and teamwork skills with residents and families
- Ability to prioritize tasks in a fast-paced long term care setting
