Roles & Responsibilities
- Provide person-centered physical therapy evaluation and treatment for skilled nursing residents
- Develop and implement individualized treatment plans that improve mobility, function, and quality of life
- Document assessments, progress notes, and discharge plans accurately and on time
- Collaborate with nursing, activities, and interdisciplinary team members to coordinate care
- Educate residents and families on home exercise programs and safe mobility strategies
- Participate in care conferences and contribute clinical expertise to resident plans
Qualifications
- Licensed or license-eligible Physical Therapist in Minnesota (DPT preferred)
- Graduates and clinicians with any level of experience are encouraged to apply — New Grads welcome
- Comfortable working in a skilled nursing facility (SNF) setting
- Strong communication skills and ability to work collaboratively with an interdisciplinary team
- Current basic life support (BLS) may be required by facility (obtainable if needed)
Education
- Doctor of Physical Therapy (DPT) or equivalent physical therapy degree; new graduates welcome
Knowledge & Skills
- Skilled in assessment and treatment planning for geriatric and post-acute populations
- Knowledge of SNF documentation, Medicare/rehab goals, and transitions of care
- Proficiency with mobility training, therapeutic exercise, and functional independence strategies
- Excellent interpersonal skills for resident and family education
- Ability to manage caseload, prioritize tasks, and maintain timely documentation
