Roles & Responsibilities

• Provide compassionate, resident-centered nursing care on each assigned shift
• Administer medications and treatments safely and accurately
• Perform regular clinical assessments and monitor changes in condition
• Document resident care thoroughly in the electronic medical record
• Support residents with activities of daily living and mobility needs
• Collaborate with nurses, therapists and support staff to deliver coordinated care
• Communicate clearly with families and the care team about resident status and needs

Qualifications

• Current Licensed Practical Nurse (LPN) license in Massachusetts required
• CPR/BLS certification required or must obtain upon hire
• Previous experience in long-term care, rehab or skilled nursing preferred
• Strong ability to follow care plans, administer medications and document care
• Reliable, professional and patient-focused with good teamwork skills

Education

• Graduate of an accredited LPN program
• High school diploma or equivalent required

Knowledge & Skills

• Clinical assessment and basic nursing care skills
• Medication administration and documentation best practices
• Familiarity with infection control and safety protocols
• Proficient communication and interpersonal skills with residents and families
• Comfortable working in a rehab and skilled nursing environment
• Ability to use electronic medical records and standard nursing documentation
