Roles & Responsibilities
- Assess patient mobility, balance, strength and functional needs using evidence-based evaluations
- Develop and implement individualized treatment plans to restore function and independence
- Provide therapeutic interventions including therapeutic exercise, gait training, and modalities as appropriate
- Document patient progress accurately and timely in the medical record, ensuring continuity of care
- Collaborate with nurses, physicians and the rehab team to coordinate comprehensive patient-centered care
- Educate patients and families on home exercise programs, safety strategies and fall prevention
- Participate in care conferences and contribute to quality improvement initiatives
Qualifications
- Current, unrestricted Physical Therapist license in California (required)
- Graduation from an accredited physical therapy program (DPT preferred)
- Current BLS/CPR certification
- Experience in skilled nursing, inpatient rehab, or post-acute settings is highly valued
- Strong documentation skills and comfort with electronic medical records
- Excellent communication skills and a collaborative bedside manner
Education
- Doctor of Physical Therapy (DPT) preferred or Bachelor's/Master's in Physical Therapy from an accredited program
Knowledge & Skills
- Knowledge of rehabilitation principles, therapeutic exercise and functional mobility training
- Skill in assessment techniques (gait, balance, transfers, ADL independence)
- Familiarity with mobility aids, orthotics and durable medical equipment recommendations
- Ability to create measurable, goal-oriented treatment plans
- Strong clinical reasoning and outcome-driven treatment approach
- Comfort working within an interdisciplinary team and mentoring support staff
