Roles & Responsibilities
- Provide patient-centered physical therapy services to residents in a skilled nursing facility
- Assess functional mobility and design individualized treatment plans that improve independence
- Deliver hands-on therapy, therapeutic exercise, and mobility training with compassion
- Collaborate with nurses, OTs, SLPs, and care teams to coordinate interdisciplinary care
- Document treatment plans, progress notes, and outcomes accurately and timely
- Educate residents and caregivers on safe transfers, home programs, and fall prevention
Qualifications
- Licensed Physical Therapist (PT) in Arizona or eligible for Arizona licensure
- Doctor of Physical Therapy (DPT) or equivalent PT degree (new grads welcome)
- Experience or strong interest in skilled nursing facility (SNF) / geriatric care
- Able to begin assignment on or around the requested start date of 2026-04-20
- Professional communication skills and ability to work well on an interdisciplinary team
Education
- Doctor of Physical Therapy (DPT) preferred; PT degree or equivalent acceptable
Knowledge & Skills
- Skilled in geriatric rehabilitation and SNF-specific therapy approaches
- Proficient with therapy documentation and care planning
- Strong clinical reasoning for mobility, balance, and fall-risk interventions
- Comfortable delivering one-on-one and group therapy sessions
- Team-oriented with good interpersonal and patient-education skills
