Roles & Responsibilities

• Provide patient-centered physical therapy services to residents in a skilled nursing facility
• Assess functional mobility and design individualized treatment plans that improve independence
• Deliver hands-on therapy, therapeutic exercise, and mobility training with compassion
• Collaborate with nurses, OTs, SLPs, and care teams to coordinate interdisciplinary care
• Document treatment plans, progress notes, and outcomes accurately and timely
• Educate residents and caregivers on safe transfers, home programs, and fall prevention

Qualifications

• Licensed Physical Therapist (PT) in Arizona or eligible for Arizona licensure
• Doctor of Physical Therapy (DPT) or equivalent PT degree (new grads welcome)
• Experience or strong interest in skilled nursing facility (SNF) / geriatric care
• Able to begin assignment on or around the requested start date of 2026-04-20
• Professional communication skills and ability to work well on an interdisciplinary team

Education

• Doctor of Physical Therapy (DPT) preferred; PT degree or equivalent acceptable

Knowledge & Skills

• Skilled in geriatric rehabilitation and SNF-specific therapy approaches
• Proficient with therapy documentation and care planning
• Strong clinical reasoning for mobility, balance, and fall-risk interventions
• Comfortable delivering one-on-one and group therapy sessions
• Team-oriented with good interpersonal and patient-education skills
