Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and treatment according to physician orders
• Monitor patient status and communicate changes promptly to the interdisciplinary care team
• Perform skilled nursing procedures such as wound care and IV documentation where applicable
• Maintain accurate, timely charting in the facility EMR and ensure clear handoffs between shifts
• Support residents’ daily living activities with dignity and respect, promoting independence whenever possible
• Collaborate with nursing leadership and allied staff to deliver high-quality, person-centered care

Qualifications

• Current and unrestricted LPN license in the state of New Hampshire (required)
• Graduate of an accredited practical nursing program
• Basic Life Support (BLS) certification required; ACLS or other specialty certifications a plus
• Ability to work the assigned weekend-focused schedule (Sat, Sun, Mon) and pick up additional shifts as needed
• Current immunizations and ability to pass background/health screenings as required by facility policy
• Previous long-term care or travel LPN experience preferred but enthusiastic, reliable clinicians are encouraged to apply

Education

• Graduate of an accredited Practical Nursing program
• Active LPN license in New Hampshire

Knowledge & Skills

• Strong medication administration and documentation skills
• Comfortable with wound care, resident assessments, and routine nursing procedures
• Familiarity with electronic medical records (EMR) systems and digital charting
• Excellent communication and teamwork skills with residents, families, and clinical staff
• Good time management and ability to prioritize in a busy long-term care setting
