Roles & Responsibilities
- Provide compassionate direct patient care, including medication administration and treatment according to physician orders
- Monitor patient status and communicate changes promptly to the interdisciplinary care team
- Perform skilled nursing procedures such as wound care and IV documentation where applicable
- Maintain accurate, timely charting in the facility EMR and ensure clear handoffs between shifts
- Support residents’ daily living activities with dignity and respect, promoting independence whenever possible
- Collaborate with nursing leadership and allied staff to deliver high-quality, person-centered care
Qualifications
- Current and unrestricted LPN license in the state of New Hampshire (required)
- Graduate of an accredited practical nursing program
- Basic Life Support (BLS) certification required; ACLS or other specialty certifications a plus
- Ability to work the assigned weekend-focused schedule (Sat, Sun, Mon) and pick up additional shifts as needed
- Current immunizations and ability to pass background/health screenings as required by facility policy
- Previous long-term care or travel LPN experience preferred but enthusiastic, reliable clinicians are encouraged to apply
Education
- Graduate of an accredited Practical Nursing program
- Active LPN license in New Hampshire
Knowledge & Skills
- Strong medication administration and documentation skills
- Comfortable with wound care, resident assessments, and routine nursing procedures
- Familiarity with electronic medical records (EMR) systems and digital charting
- Excellent communication and teamwork skills with residents, families, and clinical staff
- Good time management and ability to prioritize in a busy long-term care setting
