Roles & Responsibilities

• Perform comprehensive initial and ongoing patient assessments across physical, psychosocial, environmental and community domains
• Initiate, evaluate and modify individualized Plans of Care to meet patient needs
• Complete OASIS data and other required documentation according to federal, state and agency regulations
• Coordinate discharge planning with patients, physicians, caregivers and interdisciplinary team members
• Supervise CNA activities and ensure quality, safe care delivery
• Provide patient, family and caregiver education using appropriate teaching strategies
• Deliver care that follows SAFE error prevention habits and protects patient privacy and confidentiality
• Collaborate with other health care workers to ensure seamless, patient-centered care

Qualifications

• Active Illinois nursing license (required) — include a copy with submission
• BLS certification (required and active at time of submission)
• Home Health experience (required); strong Med-Surg experience with some Home Health will be considered
• Valid driver’s license and active car insurance (required)
• COVID and flu vaccination required (declinations will not be approved) — please add vaccination status to your profile
• OASIS and start-of-care experience strongly preferred
• Must provide resume, skills checklist (within 1 year), 2 supervisory references, active certifications and license

Knowledge & Skills

• Patient assessment and clinical judgment across the lifespan
• Initiating and modifying Plans of Care and discharge planning
• OASIS completion and start-of-care processes
• Documentation to regulatory guidelines and familiarity with federal/state home health rules
• Experience with EMR (Matrix) — charting via iPad provided
• Telehealth/virtual visit experience and comfort with remote patient monitoring
• Infusion management and wound/ostomy care skills
• Ability to supervise and coach CNAs and work within an interdisciplinary team
