Roles & Responsibilities
- Provide skilled rehabilitation interventions under the direction of the supervising physical therapist
- Deliver compassionate, person-centered care to SNF residents to improve mobility and function
- Assist with treatment plans, progress notes, and documentation to ensure quality continuity of care
- Coach residents and families on safe mobility, fall prevention, and home exercise programs
- Collaborate with nurses, therapists, and interdisciplinary team members to meet patient goals
Qualifications
- Current Physical Therapist Assistant (PTA) license or eligible for licensure in Michigan
- Graduate of an accredited PTA program
- CPR/BLS certification required or obtain upon hire
- Experience in skilled nursing facility (SNF) or geriatric populations preferred
- Strong communication skills and a team-first attitude
Education
- Associate degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Knowledge of therapeutic modalities, gait training, transfers, and ADL assistance
- Skilled in documentation and progress note completion
- Ability to follow care plans and modify treatment under PT supervision
- Familiarity with patient safety, infection control, and long-term care regulations
- Comfortable working collaboratively in an interdisciplinary setting
