Roles & Responsibilities

• Deliver therapeutic interventions under the direction of a licensed PT to help residents regain mobility and independence
• Implement treatment plans, document progress, and update the care team on patient status
• Assist with mobility, transfers, gait training and therapeutic exercises tailored to each resident
• Support fall-prevention strategies and safety measures to keep residents secure
• Educate residents and families on home exercise programs and safe functional techniques
• Collaborate with nursing and interdisciplinary staff to coordinate resident-centered care
• Maintain therapy equipment and ensure treatment areas are clean, safe, and ready

Qualifications

• Graduated from an accredited PTA program and current or eligible for state PTA licensure
• Comfortable working in a Skilled Nursing Facility (SNF) setting
• Strong communication skills and a collaborative mindset
• Any experience level welcome — new grads encouraged to apply
• Current CPR/BLS certification preferred or willing to obtain

Education

• Graduation from an accredited Physical Therapist Assistant program (Associate degree typically required); new grads considered

Knowledge & Skills

• Familiarity with SNF best practices and geriatric care principles
• Skilled in transfers, gait training, therapeutic exercise and ADL coaching
• Solid documentation skills and understanding of clinical charting
• Ability to work effectively on a multidisciplinary team
• Good clinical judgment, organization, and time management
