Roles & Responsibilities
- Deliver therapeutic interventions under the direction of a licensed PT to help residents regain mobility and independence
- Implement treatment plans, document progress, and update the care team on patient status
- Assist with mobility, transfers, gait training and therapeutic exercises tailored to each resident
- Support fall-prevention strategies and safety measures to keep residents secure
- Educate residents and families on home exercise programs and safe functional techniques
- Collaborate with nursing and interdisciplinary staff to coordinate resident-centered care
- Maintain therapy equipment and ensure treatment areas are clean, safe, and ready
Qualifications
- Graduated from an accredited PTA program and current or eligible for state PTA licensure
- Comfortable working in a Skilled Nursing Facility (SNF) setting
- Strong communication skills and a collaborative mindset
- Any experience level welcome — new grads encouraged to apply
- Current CPR/BLS certification preferred or willing to obtain
Education
- Graduation from an accredited Physical Therapist Assistant program (Associate degree typically required); new grads considered
Knowledge & Skills
- Familiarity with SNF best practices and geriatric care principles
- Skilled in transfers, gait training, therapeutic exercise and ADL coaching
- Solid documentation skills and understanding of clinical charting
- Ability to work effectively on a multidisciplinary team
- Good clinical judgment, organization, and time management
