Roles & Responsibilities

• Provide hands-on occupational therapy services under the direction of an OT to improve resident independence
• Implement and carry out individualized treatment plans and therapy goals
• Document resident progress, treatment responses, and discharge planning clearly and on time
• Collaborate with nurses, therapists, and care staff to create cohesive, person-centered care
• Educate residents and families on strategies for daily living, safety, and home programs
• Maintain a safe therapeutic environment and adhere to facility policies and regulatory standards

Qualifications

• Certified Occupational Therapy Assistant (COTA) credential required
• Eligible for or holding state licensure/registration appropriate for practice in New Hampshire
• Comfortable working in a skilled nursing facility (SNF) setting
• Open to clinicians at all experience levels, including new graduates
• Strong communication skills and ability to work effectively with an interdisciplinary team

Education

• Associate degree in Occupational Therapy Assistant or completion of an accredited OTA program preferred

Knowledge & Skills

• Skilled in ADL training, transfers, and functional task-oriented interventions
• Able to follow and implement OT treatment plans and therapeutic activities
• Proficient documentation and charting skills
• Good time management, prioritization, and teamwork abilities
• Knowledge of SNF workflows, safety protocols, and discharge planning basics
