Roles & Responsibilities

• Assess residents' communication and swallowing function using evidence-based tools
• Develop and implement individualized treatment plans that improve quality of life
• Provide compassionate, patient-centered therapy in the skilled nursing setting
• Collaborate with nursing, therapy colleagues, and families to ensure coordinated care
• Document clinical progress and FMLA-related information accurately and timely
• Educate residents and caregivers on strategies to support carryover and safety
• Participate in care planning and interdisciplinary meetings to optimize outcomes

Qualifications

• ASHA CCC-SLP (required)
• Active or eligible SLP license for Tennessee
• 1-3 years experience working as an SLP, experience in SNF/geriatric care preferred
• Comfortable documenting and supporting FMLA processes as required by facility
• Strong interpersonal skills and ability to collaborate with interdisciplinary teams

Knowledge & Skills

• Expertise in dysphagia assessment and management in older adults
• Knowledge of cognitive-communication disorders and therapy approaches
• Familiarity with SNF documentation standards and regulatory requirements
• Skill in developing measurable, functional treatment plans
• Ability to communicate clinical findings clearly to families and team members
