Roles & Responsibilities
- Prepare and compound sterile IV medications, including hazardous and non-hazardous agents, following strict sterile technique
- Safely compound chemotherapy and antineoplastic agents with attention to packaging and handling protocols
- Verify IV preparations and medication orders to ensure accurate dosing and patient safety
- Maintain aseptic conditions and perform required PATT testing and documentation
- Collaborate with nursing and clinical teams to support pharmacotherapy and therapeutic drug monitoring
- Operate and troubleshoot compounding equipment such as Baxter systems and Cad cassette setups
- Ensure compliance with state regulations and facility policies, including controlled substance handling and recordkeeping
- Provide clinical input on dosing, IV piggyback/push administration, and high-alert medication management
Qualifications
- PharmD (required)
- Active California Pharmacist License (required)
- Minimum 2 years experience in sterile compounding/IV room (hazardous and non-hazardous drugs)
- Certifications: BLS preferred; Chemo/Hazardous Drug Certification and IV Certification preferred
- Must pass PATT testing as required by facility
- Experience with Baxter systems, Cad cassette, and medication dispensing systems preferred
- Comfort floating to nearby sister facilities within walking distance as needed
Knowledge & Skills
- Expertise in sterile compounding and aseptic technique
- Chemotherapy compounding and safe handling of hazardous drugs
- IV preparation skills including IV piggyback and IV push medications
- Familiarity with Baxter compounding systems and Cad cassette operations
- Strong pharmacotherapy management, dosing, and therapeutic drug monitoring
- Attention to detail for high-alert medications and packaging antineoplastic agents
- Ability to document accurately and follow facility compounding policies
