Roles & Responsibilities
- Provide compassionate, hands-on nursing care for residents in a long-term care setting
- Administer medications and treatments accurately and document care per facility policy
- Collaborate with the interdisciplinary care team to develop and follow individualized care plans
- Perform timely and thorough charting, including MDS-related documentation when applicable
- Monitor resident status and report changes to clinical leadership for prompt intervention
- Support resident dignity and quality of life through person-centered care and communication
Qualifications
- Current, unrestricted Colorado LPN license (required)
- Long-term care (LTC) experience (required)
- MDS (Minimum Data Set) experience (preferred)
- Active clinical skills in medication administration, wound care, and resident assessments
Education
- Graduate of an accredited LPN/LVN program or equivalent practical nursing education
Knowledge & Skills
- Skilled in clinical documentation and charting, including familiarity with MDS processes
- Strong observational skills and ability to prioritize resident needs
- Excellent communication and teamwork within a long-term care environment
- Comfortable with time management for high-quality, efficient resident care
- Ability to follow facility policies, infection control standards, and safety protocols
