Roles & Responsibilities
- Provide compassionate, hands-on nursing care to long term care residents
- Administer medications and treatments safely and according to policy
- Perform resident assessments and report changes in condition promptly
- Complete accurate documentation, including MDS-related entries when applicable
- Collaborate with the interdisciplinary care team to implement care plans
- Support residents with activities of daily living while preserving dignity
- Assist with admissions, transfers, and discharges as needed
- Promote a safe, comfortable environment and adhere to infection control practices
Qualifications
- Current, unrestricted Colorado LPN license required
- Long Term Care (LTC) experience required
- MDS experience preferred
- Strong clinical judgment and ability to prioritize resident needs
- Excellent communication and teamwork skills
- Ability to work the posted day shift hours reliably
Education
- Completion of an accredited Licensed Practical Nurse (LPN) program or vocational nursing diploma
Knowledge & Skills
- Knowledge of long term care nursing practices and resident-centered care
- Familiarity with MDS processes and documentation best practices
- Medication administration and basic wound care skills
- Accurate charting and clinical documentation abilities
- Strong interpersonal skills for working with residents, families, and staff
