Roles & Responsibilities

• Assess and evaluate residents to create individualized treatment plans that improve mobility and independence
• Deliver skilled physical therapy interventions in a skilled nursing facility (SNF) setting
• Document clinical progress and outcomes clearly and timely to support continuity of care
• Collaborate with nursing, therapy colleagues, and interdisciplinary teams to coordinate resident-centered care
• Educate residents and families on therapeutic goals, home exercise programs, and fall prevention strategies
• Participate in facility meetings and quality initiatives to enhance patient care and operational workflows

Qualifications

• PT or DPT credential (Physical Therapist)
• Any level of experience welcomed, including new graduates — the posting explicitly states new grads may apply
• Comfortable providing care in a Skilled Nursing Facility (SNF) practice setting
• Ability to work day shifts and adapt to facility scheduling
• Professional communication skills and a team-first attitude

Education

• DPT or PT degree (new graduates encouraged to apply)

Knowledge & Skills

• Foundational physical therapy assessment and treatment skills relevant to geriatric and SNF populations
• Strong documentation and clinical reasoning to track resident progress
• Ability to collaborate with interdisciplinary teams to implement comprehensive care plans
• Time management and prioritization in a fast-paced SNF environment
• Resident-centered care approach with excellent interpersonal skills
