Roles & Responsibilities
- Assess patients in their home environment and create personalized treatment plans
- Deliver hands-on and therapeutic exercise interventions to improve mobility and independence
- Educate patients and caregivers on safe transfers, home exercise programs, and fall prevention
- Document skilled care accurately and timely in the electronic medical record
- Collaborate with nurses, social workers, and other clinicians to support holistic patient care
- Monitor patient progress and modify plans of care to meet functional goals
- Assist with discharge planning and community resource coordination to support transitions
Qualifications
- Active Physical Therapist license in the state of practice (or ability to obtain quickly)
- Current BLS/CPR certification
- Home health experience preferred but enthusiastic new grads with strong clinical skills will be considered
- Reliable transportation and ability to travel between patient homes
- Excellent communication and time-management skills to manage a caseload effectively
Education
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) from an accredited program
Knowledge & Skills
- Skilled in musculoskeletal, neurological, and geriatric rehabilitation approaches
- Proficient with home safety assessments and adaptive equipment recommendations
- Strong documentation skills and comfort using electronic medical records
- Ability to prioritize caseloads and work independently in the field
- Compassionate patient education and caregiver coaching abilities
