Roles & Responsibilities

• Conduct comprehensive clinical screening, assessment, diagnosis, and treatment planning across the lifespan
• Provide brief therapy and continuing care for patients from birth through elder
• Facilitate or co-facilitate patient care conferences and lead discharge planning for ALF or transitional placements
• Coordinate care with hospital teams and community partners to ensure smooth transitions across the continuum
• Advocate for clients and families, assisting with complex matters such as Power of Attorney when relevant
• Maintain accurate, timely, and compliant documentation in alignment with state, federal, accreditation, and hospital standards
• Support chemical dependency and mental health needs with appropriate referrals and clinical recommendations
• Participate in quality management activities, staff trainings, provider enrollment and assigned billing duties

Qualifications

• Masters Degree in Social Work, Psychology, Counseling, or a related discipline (required)
• Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT) licensed or in process
• Valid State of Alaska driver license or obtainable within six months of hire
• Must be 21 years of age
• Clinical competency assessment required within 3 months of hire and every three years
• Experience with discharge planning, care coordination, case management, and documentation best practices

Education

• Masters Degree in Social Work, Psychology, Counseling or related discipline from an accredited institution

Knowledge & Skills

• Skilled in interviewing techniques and crisis intervention
• Strong understanding of ethical and legal management practices and patient rights
• Proficient with documentation regulations tied to accreditation and hospital billing requirements
• Ability to observe, record, and evaluate psychiatric and substance use behaviors
• Excellent written communication for clear, concise reports and chart notes
• Collaborative approach for multidisciplinary team work and community partnerships
