Roles & Responsibilities
- Conduct comprehensive clinical screening, assessment, diagnosis, and treatment planning across the lifespan
- Provide brief therapy and continuing care for patients from birth through elder
- Facilitate or co-facilitate patient care conferences and lead discharge planning for ALF or transitional placements
- Coordinate care with hospital teams and community partners to ensure smooth transitions across the continuum
- Advocate for clients and families, assisting with complex matters such as Power of Attorney when relevant
- Maintain accurate, timely, and compliant documentation in alignment with state, federal, accreditation, and hospital standards
- Support chemical dependency and mental health needs with appropriate referrals and clinical recommendations
- Participate in quality management activities, staff trainings, provider enrollment and assigned billing duties
Qualifications
- Masters Degree in Social Work, Psychology, Counseling, or a related discipline (required)
- Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT) licensed or in process
- Valid State of Alaska driver license or obtainable within six months of hire
- Must be 21 years of age
- Clinical competency assessment required within 3 months of hire and every three years
- Experience with discharge planning, care coordination, case management, and documentation best practices
Education
- Masters Degree in Social Work, Psychology, Counseling or related discipline from an accredited institution
Knowledge & Skills
- Skilled in interviewing techniques and crisis intervention
- Strong understanding of ethical and legal management practices and patient rights
- Proficient with documentation regulations tied to accreditation and hospital billing requirements
- Ability to observe, record, and evaluate psychiatric and substance use behaviors
- Excellent written communication for clear, concise reports and chart notes
- Collaborative approach for multidisciplinary team work and community partnerships
