Roles & Responsibilities
- Conduct thorough psychosocial assessments to understand patients’ strengths, needs, and goals
- Develop individualized treatment plans that are practical, evidence-based, and person-centered
- Deliver evidence-based therapeutic interventions across the lifespan for issues like depression, anxiety, trauma, and substance use
- Coordinate care closely with primary care providers, nurses, and other behavioral health professionals to ensure whole-person support
- Maintain clear, timely clinical documentation to support continuity of care and quality outcomes
- Engage and advocate for underserved and rural populations to improve access and care equity
- Participate in team meetings and contribute to quality improvement initiatives that enhance patient care
Qualifications
- Active LISW license in Iowa (required)
- Master's degree in Social Work from an accredited program (MSW)
- Minimum of 2 years of clinical experience working with behavioral health populations preferred
- Experience or interest in serving rural or medically underserved communities is a strong plus
- Strong interpersonal, organizational, and documentation skills
- Commitment to evidence-based practice and collaborative, integrated care
Education
- Master's in Social Work (MSW) from an accredited program
Knowledge & Skills
- Skilled in psychosocial assessment and treatment planning
- Experience delivering evidence-based therapeutic interventions (e.g., trauma-informed care, CBT approaches)
- Comfort working across the lifespan with diverse behavioral health needs
- Solid care coordination abilities and collaborative teamwork
- Proficient clinical documentation and use of electronic health records
- Cultural humility and experience serving underserved populations
