Roles & Responsibilities

• Provide skilled physical therapy interventions to assess, treat, and progress patients toward functional goals
• Develop personalized treatment plans using evidence-based practice and clear documentation
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Educate patients and families on therapeutic techniques, home exercise programs, and safety strategies
• Monitor patient progress, modify treatment as needed, and communicate outcomes to the care team
• Maintain timely, accurate documentation in the facility electronic health record

Qualifications

• Active or eligible state license as a Physical Therapist (New Hampshire preferred or ability to obtain)
• Current Basic Life Support (BLS) certification or willingness to obtain promptly
• Clinical experience in outpatient, inpatient, or rehab settings preferred
• Strong assessment, manual therapy, and therapeutic exercise skills
• Excellent communication and teamwork abilities; patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy considered

Knowledge & Skills

• Skilled in musculoskeletal and neuromuscular evaluation and treatment
• Proficient in creating and updating individualized treatment plans
• Comfortable with documentation best practices and electronic charting
• Knowledgeable about functional mobility, gait training, and assistive device prescription
• Ability to educate patients and caregivers with clarity and compassion
