Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and on time
• Monitor patient status and communicate changes to the care team
• Document care, observations, and patient responses clearly and promptly
• Collaborate with fellow nurses and interdisciplinary team members to ensure continuity of care
• Support patients and families with education, empathy, and clear communication

Qualifications

• Active Licensed Practical Nurse (LPN) credential — required (must be eligible to practice in Pennsylvania)
• Basic Life Support (BLS) certification — required or willing to obtain promptly
• Current clinical experience in a similar care setting is preferred
• Strong communication skills and a patient-centered attitude
• Ability to pass background and health screenings required by the facility

Education

• Diploma or certificate in Practical Nursing (LPN) or equivalent
• High school diploma or GED

Knowledge & Skills

• Clinical skills in medication administration and basic patient assessments
• Familiarity with charting/documentation and care planning
• Ability to prioritize tasks in a fast-paced environment
• Good interpersonal skills for working with patients, families, and the care team
• Basic wound care and monitoring skills
