Roles & Responsibilities

• Provide direct patient care with compassion and clinical accuracy
• Administer medications and treatments per orders and facility protocols
• Perform thorough and timely documentation in the patient record
• Monitor and report changes in patient condition to the care team
• Collaborate with nurses, therapists, and providers to deliver coordinated care
• Assist with activities of daily living while promoting patient dignity and independence
• Support infection control practices to maintain a safe care environment

Qualifications

• Current, active LPN license (eligible to practice in Pennsylvania)
• CPR/BLS certification (or willingness to obtain prior to start)
• Ability to pass background check and drug screening as required
• Comfortable working in a fast-paced long-term care/rehab environment
• Professional communication skills and a patient-centered attitude

Education

• Completion of an accredited Practical Nursing (LPN/LVN) program or equivalent

Knowledge & Skills

• Strong fundamentals in vital signs, basic assessments, and charting
• Medication administration and documentation accuracy
• Basic wound care and pressure injury prevention
• Familiarity with standard infection control practices
• Teamwork, time management, and a patient-first mindset
• Comfort using electronic medical records for documentation
