Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and basic nursing treatments
• Monitor patients’ conditions and report changes promptly to the care team
• Collaborate with RNs, physicians, and interdisciplinary staff to implement care plans
• Document assessments, interventions, and patient responses accurately and timely
• Support patients with activities of daily living while promoting dignity and independence
• Participate in shift handoffs to ensure continuity and safety of care

Qualifications

• Active LPN (Licensed Practical Nurse) license in the appropriate state or compact privileges as required
• Previous experience in long-term care, skilled nursing, or similar settings preferred
• Current Basic Life Support (BLS) certification or willingness to obtain
• Up-to-date immunizations and the ability to pass background and health screenings
• Strong communication skills and a team-oriented mindset
• Flexibility to work night shifts for the duration of the 13-week assignment

Education

• Diploma or certificate in Practical Nursing (LPN) from an accredited program

Knowledge & Skills

• Clinical skills in medication administration, wound care basics, and patient assessment
• Charting and documentation best practices, including electronic health record familiarity
• Ability to prioritize, multitask, and respond to urgent changes in patient status
• Patient-centered care approach with strong interpersonal skills
• Basic knowledge of infection control and safety protocols
