Roles & Responsibilities

• Provide compassionate, high-quality direct patient care following facility policies
• Administer medications and treatments accurately and document appropriately
• Monitor and record vital signs, report changes, and escalate concerns when needed
• Collaborate with RNs, providers, and interdisciplinary team members to support patient care plans
• Assist patients with activities of daily living while promoting independence and dignity
• Maintain clear, timely charting in the electronic medical record and handoff communications
• Adhere to infection control standards and ensure a safe care environment
• Support onboarding of new team members and contribute to a positive workplace culture

Qualifications

• Current LPN license in good standing (NH licensure or compact where applicable)
• Basic Life Support (BLS/CPR) certification or ability to obtain prior to start
• Comfortable working in long-term care / facility environment (experience preferred)
• Strong communication and teamwork skills with a patient-centered approach
• Ability to pass background check and required immunizations per facility policy

Education

• Graduate of an accredited Practical Nursing program (LPN/LVN diploma)

Knowledge & Skills

• Medication administration and documentation best practices
• Skilled in taking and interpreting vital signs and basic nursing assessments
• Familiarity with electronic medical records and charting workflows
• Excellent patient communication, family education, and bedside manner
• Time management and prioritization in a fast-paced clinical setting
