Roles & Responsibilities

• Provide compassionate, hands-on care to residents including assistance with activities of daily living
• Monitor and record vital signs and report changes in condition to the nursing team
• Support safe transfers and mobility, using proper body mechanics and assistive devices
• Maintain accurate, timely documentation in accordance with facility policy
• Practice infection prevention and maintain cleanliness of resident environments
• Collaborate with RNs and interdisciplinary team members to deliver person-centered care
• Build trusting relationships with residents and families to promote well-being and dignity

Qualifications

• Current LNA or T-LNA certification for New Hampshire (VT credential accepted if noted)
• Completion of a state-approved nursing assistant program
• Current CPR/BLS certification preferred or willingness to obtain
• Ability to pass background check and required health screenings or immunizations
• Compassionate attitude, strong communication skills, and reliable work ethic

Education

• High school diploma or equivalent
• Completion of an approved Nursing Assistant program and state certification

Knowledge & Skills

• Skilled in resident-centered care and assisting with ADLs
• Basic knowledge of infection control and safety practices
• Ability to measure and document vital signs accurately
• Good communication and teamwork skills in a fast-paced long-term care setting
• Familiarity with charting and following care plans
